Improving radiotherapy after breast-conserving surgery.

چکیده
Drs. Mouw and Harris have reviewed data from randomized clinical trials (RCTs) and from selected institutional experiences with whole breast irradiation (WBI), usually delivered with tangent pair radiation therapy (RT) over 5 to 7 weeks, accelerated whole-breast irradiation (AWBI), usually delivered over 3 to 4 weeks, and accelerated partial-breast irradiation (APBI), usually delivered in a single treatment or via up to a week of twice-daily treatments.[1] The authors conclude that the RCTs demonstrate that AWBI is equivalent to “standard” WBI in terms of 5and 10-year local control, cosmetic outcomes, and breast cancer–specific and overall survival. There is much less long-term data supporting the equivalence of APBI and WBI. The authors are correct to advise caution in selecting patients for APBI, and they emphasize that true rates of long-term morbidity will only be known as a result of ongoing follow-up of patients treated with various APBI techniques.

Compare the organs at risk in breast-conserving three- dimensional conformal radiotherapy (3D-CRT) based on patient's breast size
Introduction: Breast cancer is the most common cancer among women. Radiotherapy plays an important role in the treatment of breast cancer and essential part of the standard treatment after breast-conserving surgery, routinely done to eliminate the microscopic residual cancer. The heart and lungs are one of the most important organs at risk of breast cancer radiotherapy that th...

متن کامل10 year survival after breast-conserving surgery plus radiotherapy compared with mastectomy in early breast cancer in the Netherlands: a population-based study.
BACKGROUND Investigators of registry-based studies report improved survival for breast-conserving surgery plus radiotherapy compared with mastectomy in early breast cancer. As these studies did not present long-term overall and breast cancer-specific survival, the effect of breast-conserving surgery plus radiotherapy might be overestimated. In this study, we aimed to evaluate 10 year overall an...
متن کاملRole of postmastectomy radiation therapy after neoadjuvant chemotherapy in locally advanced breast cancer.
Neoadjuvant chemotherapy for primary breast cancer is the gold standard in the treatment of locally advanced, inoperable breast cancer, and also based on a large body of evidence has become a standard treatment option for patients with operable disease, who are clear candidates for adjuvant chemotherapy. There are several advantages of administering neoadjuvant chemotherapy: tumor downstaging, ...
متن کاملComparison of “heart and lung volume absorbed dose” between electron and photon boost radiotherapy after breast conserving surgery
Background: Breast conserving surgery (BCS) followed by radiotherapy (RT) has been widely accepted as the standard treatment in early stages of invasive breast cancer. The standard technique of RT includes whole breast irradiation (WBI). Additional tumor bed boost irradiation has also an important role in the local tumor control. But there are various controversial delivery methods. The aim of ...
متن کاملImpact of interval from breast conserving surgery to radiotherapy on local recurrence in older women with breast cancer: retrospective cohort analysis
OBJECTIVES To determine if the length of interval between breast conserving surgery and start of radiotherapy affects local recurrence and to identify factors that might be associated with delay in older women with breast cancer. DESIGN Retrospective cohort analysis with Cox proportional hazards models to study the association between time to radiotherapy and local recurrence, and propensity ...
